Sixth Grade Immunizations
Incoming Sixth Graders are required to have certain immunizations by the first day of school. See attached letter for more information.
Kumpf School Nurse
According to the NJ state guidelines, a yearly health screening is conducted for each student in Kindergarten through grade 12, including; blood pressure, height, weight, visual acuity, auditory acuity, and scoliosis. A note will be sent home by the nurse of any deviations from the recommended standards and you will be asked to follow up with your child's physician. If a physical is submitted yearly by your child's primary care physician your child may not need to be screened that school year.
If you do not want your child to be screened, please notify the school nurse in writing in the beginning of the school year.

Sports Physicals
Any students in grades 7 and 8 wishing to try-out for interscholastic sports teams must complete the Family ID forms and have a valid sports physical. See the link below.
